The new lieutenant governor wants Medicaid expansion. The Senate she will run is still Republican. Before anybody votes, East Texas deserves the math.

Plain English summary

Lt. Gov.-elect Vikki Goodwin told CBS News Texas she wants to work on Medicaid expansion when she takes the gavel. She said five million Texans have no health insurance. She said expansion would bring federal dollars back to Texas. CBS reports Republicans have rejected expansion before and keep 18 of the 31 Senate seats. (DOCUMENTED INFERENCE: CBS News Texas, November 8, 2026.)

The regular session starts January 12, 2027. (FACT)

So this is not a campaign line anymore. It is an agenda item for the person who will preside over the Texas Senate.

Why it matters

The lieutenant governor is the President of the Senate. That is in the Texas Constitution. (FACT)

When the person holding the gavel says a topic is on the table, it gets talked about. That is how Austin works.

Medicaid expansion is one of the biggest money decisions Texas can make. Federal law sets the federal share for the expansion group at 90 percent for 2020 and later. (FACT) The state carries the rest. Ten percent of a big number is still a big number.

Out here, people know what a closed hospital looks like. CBS reports Goodwin wants to hear from senators whose rural areas have seen a hospital close. (DOCUMENTED INFERENCE) That is an East Texas conversation whether Austin admits it or not.

The receipt

  • FACT: Texas Government Code, Section 301.001. The Legislature meets in regular session on the second Tuesday in January of odd-numbered years. In 2027 that is January 12.
  • FACT: Texas Constitution, Article 4, Section 16. The lieutenant governor is President of the Senate.
  • FACT: Texas Constitution, Article 3, Section 2. The Senate has 31 members.
  • FACT: 42 U.S.C. 1396d(y)(1). The federal share for newly eligible adults is 90 percent for 2020 and each year after.
  • DOCUMENTED INFERENCE: CBS News Texas, November 8, 2026, quotes Goodwin: "We have five million Texans without health insurance." And: "I think that, yes, there's an opportunity there, and I will work on that."
  • DOCUMENTED INFERENCE: CBS News Texas reports Republicans hold 18 of 31 Senate seats and have rejected expansion.
  • NEEDS AUTHENTICATION: the five million figure. It is her number as reported. I have not matched it to a Census table. Show the source.

What Ryan says

RYAN STATEMENT: I am not going to tell you this is good or bad before I see the numbers. That is the whole point.

Here is what I want on the record before one vote:

  • The cost to the state, year by year.
  • How many people in Gregg, Harrison, Upshur, Smith and Marion counties it would cover.
  • What it pays rural hospitals, by name.
  • What happens to the state share if Washington changes the deal.
  • Who wrote the bill, and who paid for the study behind it.

Same rule I asked for on vouchers. Show the audit before the vote.

Same rule, no matter whose idea it is. If a Republican brought this, I would ask the same five questions.

If it keeps a hospital open in a small East Texas town, prove it. If it blows a hole in the budget, prove that too. The record does not lie, if somebody bothers to write it down.

And to the Republicans holding 18 seats: say no if you want. Say why, with numbers. "We rejected it before" is not a reason. It is a habit.

What this does not prove

It does not show a filed bill. This article cites none.

It does not prove the Senate will take it up or pass it.

It does not prove the five million number.

It does not prove expansion would save, or fail to save, any particular hospital.

It does not prove what expansion would cost Texas.

What it does prove

The incoming President of the Senate said on the record, as reported by CBS, that she will work on Medicaid expansion.

The session date is set in state law. The federal share is set in federal law.

The math can be done. Somebody just has to publish it.

The bottom line

Big promise. Big money. Republican Senate. East Texas hospitals in the middle.

Show the math. Then vote.
